Evolent Care Partners and Physicians Accountable Care of Utah Partner to Deliver More Personalized, Coordinated Care

Washington, D.C., December 16, 2020 – Evolent Care Partners, a network of independent primary care providers focused on total cost of care management, announced a new partnership with Physicians Accountable Care of Utah (PACU). Beginning in January 2021, PACU will participate with The Accountable Care Organization, Ltd. (TACO) in the Medicare Shared Savings Program (MSSP) Enhanced Track.

PACU is an ACO founded in 2013 by Granger Medical Clinic and consists of leading independent medical groups Granger Medical Clinic, Ogden Clinic and Utah Gastroenterology. Granger Medical Clinic and Ogden Clinic will be joining TACO. They are committed to creating a better health care experience for patients by working in a more collaborative fashion. With this partnership, these groups join a growing national network of independent primary care providers that currently cares for over 60,000 Medicare beneficiaries across Indiana, Michigan, North Carolina, and Texas. The collaboration will add 185 primary care providers in Utah and 10,000 original Medicare beneficiaries to Evolent Care Partners’ ACO, a reflection of its continued market expansion.

With the partnership between Evolent Care Partners and PACU, TACO will be the largest MSSP network in Utah that is comprised solely of independent providers. MSSP’s Enhanced Track also offers providers the opportunity to capture more savings than they otherwise would in MSSP’s Basic Track. Through a unique partnership model, physicians achieve success by focusing on primary and preventive care services, referring to high-quality specialists, and keeping Medicare beneficiaries healthy.

Similarly, this collaboration establishes a foundation for the network as it looks to partner beyond MSSP and manage other health populations in value-based care arrangements, including Medicare Advantage and commercial health plans.
