Who You’ll Be Working With:

Evolent Health is looking for a Director, Clinical Delivery to join us as a key member of the Clinical Delivery team. Reporting to the Vice President of Clinical Delivery, the Director will play a critical role in the design, innovation, and deployment of new and existing clinical initiatives, serve as a key strategic leader and change agent within the organization, and act as a catalyst for the execution of our clinical model across a variety of client engagements.

The Director will function in a dyad model, collaborating closely with a clinical SME counterpart, to execute on internal and client-facing Clinical Delivery efforts. The Director will also work cross-functionally with many teams, including Partner Delivery, RAF/STARS, Clinical Research & Development, Quality, Clinical Network Performance, and Clinical Programs & Performance to deliver best-in-class population health initiatives which includes the perspective of clinical end users across the health care continuum.  The Director is expected to lead multiple engagements in parallel and will be responsible for determining courses of action in complex and undefined situations that may not be addressed by existing policies, procedures, or protocols. 

What You’ll Be Doing:


  • Facilitate the design, productization, and deployment of innovative value-based care initiatives across the health care continuum, including work with care management programs, risk adjustment, IT applications such as Identifi Practice, MA Stars and innovation pilot
  • Continuously improve Evolent clinical solutions to exceed client expectations and differentiate Evolent from competitors
  • Develop culture of high performance, innovation, accountability, and continuous improvement that values learning, emphasizes the importance of patient experience, and commitment to quality
  • Collaborate with all departments within the company relating to innovation, quality and process improvement to drive best practice and knowledge sharing
  • Develop and iterate a reproducible process for completion of assessments, development of recommendations, and shaping of client deliverables
  • Work with internal Evolent teams to define analytics needs and Key Performance Indicators to measure outcomes and success of population health initiatives

Strategic Project Management and Leadership:

  • Provide strategic project management and leadership for multiple client and internal engagements simultaneously
  • Serve as lead project manager for clinical workstreams, with ability to adapt quickly to changing priorities and timelines
  • Working closely with clinical counterpart, drive error-free content development of high quality client-facing and internal deliverables under tight timelines; may include project plans, current state assessments, implementation plans, executive summaries, presentations, etc.
  • Build and maintain work plans: ensure deliverables meet client objectives; include appropriate time for review cycles with relevant parties; account for key project interdependencies
  • Manage work to plan, escalating issues where appropriate, and driving resolution in areas of risk
  • Serve as a liaison to other engagement workstreams and engagement leadership by actively supporting, coordinating, and sharing learnings across the partner delivery model
  • Structure, create, finalize, and deliver effective presentations tailored to all levels of the client organization – including executive, management, clinical staff, physician/provider staff, and information technology resources – to effectively convey strategy and outcomes
  • Lead and support business development opportunities (e.g., bus dev conversations, support of sales presentations, RFPs, and SOW s, etc.), particularly to enhance Evolent’s overall business and sales

Consultant Subject Matter Expertise:

  • Serve as internal Consultant Subject Matter Expert in the analysis, review and design of clinical delivery work, including clinical delivery models, practice transformation, care management programs, network optimization, workflow enhancement, government policies and Evolent offerings
  • In undefined and dynamic situations, apply strategic decision-making frameworks to assess and frame problems to support our partners through the transformative process from fee-for-service to value-based care
  • Proactively anticipate and identify potential roadblocks and take a solutions-driven approach to escalate, manage, address, and resolve
  • Persuades and affects change in both group and one-on-one situations; works with challenging clients to navigate through difficult issues
  • Build credibility and maintain strong professional relationships with client and internal stakeholders through regular contact during course of engagement
  • Understand Evolent’s capabilities and limitations in order to build viable solutions for client, rapidly build knowledge in areas of unfamiliarity as needed, and develop executable solutions for client

  • In partnership with the Clinical Senior Director/Director counterparts, lead the successful implementation of Evolent’s model of care, including but not limited to:
    • Care management clinical programs (including workflow and staffing)
    • Use of Identifi platforms to deliver model of care (in partnership with Partner Delivery)
    • Physician and network governance
    • Practice and physician communications and engagement (in partnership with Communications)
    • Quality
    • Risk Adjustment workflow and adoption
  • Support clinical assessment of partners’ network and practice current state , and identify high-value opportunities and needs for operational success in a value-based model of care
  • Support relationship-building with physicians and practice management in the market to solve operational challenges and build trust within the markets
  • Support the development of practice educational programs and engagement strategies
  • Support analytics in development of operational and cost performance reports designed to track progress, engage primary care providers in performance improvement and track ROI
  • Provide clear and actionable communications (verbal and written) to the client and engagement team
  • Lead codification of existing deliverables and processes to facilitate ease of adoption by market operations teams and reproducibility for future Evolent partners

The Experience You Need (Required):


  • 5 to 7 years of experience in a Healthcare space
  • Experience in consulting, services or population health organizations with a proven ability to manage partner relationships and achieve change within another organization from assessment through implementation
  • Knowledge of value-based care models, practice operations and workflows, risk adjustment, accountable care organizations, and Medicare Advantage quality initiatives
  • Experience launching large scale initiatives, preferably in ambulatory and/or inpatient settings
  • Proactive, research-driven approach to problem solving
  • Ability to translate complex clinical and operational concepts into a cohesive, visual story
  • Ability to rapidly learn and adapt to ambiguous environments and clientele with quickly changing priorities and timelines
  • Strong history of quickly building relationships, gaining credibility, influencing and partnering with business and clinical leaders and peers across the organization
  • Ability to partner with multiple constituents, manage in a matrix relationship environment, strengthen and deliver on our agenda, and drive positive change in a high performance settings
  • Excellent presentation and facilitation skills in both large and small scale settings
  • Excellent written and verbal communication skills and the ability to communicate/collaborate effectively with all levels of employees, management, executives, providers, and external organizations
  • Excellent proficiency in the Microsoft Office Suite, in particular Microsoft PowerPoint, Word, and Excel
  • Willingness to travel up to 50%

Finishing Touches (Preferred):

  • Masters in health or business-related discipline preferred
  • Practical experience in physician practice settings
  • Implementation of initiatives relevant to value-based care models, practice operations and workflows, risk adjustment, accountable care organizations, or Medicare Advantage quality initiatives
