The independent provider’s ACO  

Many independent primary care providers are attracted to accountable care organizations (ACOs) by the promise of delivering better, more holistic care for patients while increasing practice revenue. Yet too often, they don’t feel equipped to take the leap into the two-sided performance-based contracts.  

Through our Primary Care Solution (Evolent Care Partners), Evolent provides the capital, resources and technology to help independent primary care providers succeed in value-based care, without assuming downside risk or out-of-pocket expenses. Operating since 2020 as Evolent Care Partners, our Medicare Shared Savings Program Enhanced Track ACO now has more than 1,200 providers and has saved Medicare over $100 million since inception.  

Highlights of our ACO solution include: 

New and immediate income opportunity 

Our ACO rewards providers with immediate and potential long-term revenue streams, including:

  • Network sign-on pre-payment for committing to the ACO and performing agreed-upon services  
  • Quarterly payments for activities that drive quality improvement and advance ACO initiatives
  • Enhanced Track shared savings, with the majority of shared savings captured by providers after the ACO’s operating expenses are covered 
  • Qualification for Part B Advanced Alternative Payment Model bonus 

Population health capabilities  

Practices gain no-cost access to population health tools and proven care coordination resources, including:

  • Care coordination team that delivers proven clinical programs to improve outcomes for patients who are Medicare beneficiaries
  • Proprietary Panel Insight platform that identifies most high-impact population health activities for the Medicare beneficiaries and prompts practices to complete them
  • Analytics to identify local cost and quality improvement opportunities

Evolent assumes financial responsibility for network performance, meaning that providers:
