Our network model generates immediate and ongoing income for existing networks and their providers, while ensuring physicians capture the majority of future value.
For Independent Primary Care Physicians
We offer participation in our ACO to qualifying individual primary care physicians and smaller practices in our existing markets. Participating providers are eligible for activity-based incentives paid during the performance year, as well as Medicare Part B fee-schedule bonuses and shared savings payments.
